Unique visit, account, or encounter number.
dob
Date of birth in standard date format (e.g. YYYY-MM-DD or MM/DD/YYYY).
sex
Patient biological sex: M or F.
admit_date
Inpatient hospital admission date.
icd10_dx
Principal and secondary ICD-10-CM diagnosis codes.
Accepted formats: L97.811, l97.811, L97811.
Present On Admission (POA) Flags (Optional): Defaults to E if omitted.
- Y: Present at the time of inpatient admission.
- N: Not present at admission.
- U: Documentation insufficient to determine POA.
- W: Clinically undetermined.
- E: Exempt from POA reporting.
Optional Columns
icd10_pcs
ICD-10-PCS inpatient procedure codes (e.g. 5A1955Z, 0210093).
discharge_date
Inpatient discharge date for length-of-stay calculation.
disposition
Discharge status code. Defaults to 01 (Discharged home) if discharge date is provided, or 30 (Still a patient) if omitted.
